COVID-19 Vaccination Clinics

Eligible New Yorkers are:  Ages 5 and older*, reside, work or study in New York State

*Pfizer is the only COVID-19 vaccine that has emergency use authorization for 5-17 year olds.  Any minor seeking vaccination must be accompanied by a parent/legal guardian whose consent will be obtained at time of vaccination.  If a parent/guardian is unable to accompany the child, he/she may designate another adult to consent on their behalf.  Such designation must be in writing and must include the name and date of birth of the child, the name and signature of the parent/guardian, and the name of designee.  Photo identification will be required.   Minor authorization form

Additionally, there will be a Youth Vaccine Clinic for Northern Chautauqua County Youth, sponsored by the Boys & Girls Club of Northern Chautauqua County, The Chautauqua Center, and the United Way of NCC on Thursday, February 3rd at the Boys & Girls Club of NCC Building in Dunkirk at the old DCSD School #4, 752 Central Ave, Dunkirk, NY.

They will have pediatric vaccine doses available for youth ages 5-11, as well as Pfizer 12+ doses for teens or to use as boosters.

There will also be an Information Session for parents provided on Thursday, January 27th at 8:30 pm via Facebook Live Watch Party from the BGC NCC Facebook page with a local doctor from the Chautauqua Center to go over FAQ's about vaccinating children and answer any questions/address concerns.
