Drive-Thru Vaccination Clinic Scheduled

On Tuesday, May 11th, the Chautauqua Health Department will be holding a Covid Vaccination Clinic at the Cassadaga Valley Central School Bus Garage.  This clinic will provide Pfizer vaccine to anyone 16 years of age or older and/or Moderna vaccine to anyone 18 years of age or older.  Anyone over 18 will have their choice of vaccines, anyone 16-18 will have to get Pfizer. The clinic will take place in the bus garage and will run from 4:30 pm until 6:30 pm.  The clinic for the second dose for both vaccines will be Tuesday June 8th again from 4:30 pm until 6:30 pm at the bus garage.  This second dose date falls within the guidelines for both vaccines and makes it so that we only need to have families come back once if they pick two different vaccines.

to pre-register for an appointment, or we will be accepting walk-ins/those with no appointment. Anyone under 18 years of age must have a signed copy of this consent form.  (Click on "consent form" or go here for the form:  https://5il.co/sdl8 ) Students can either turn in that consent to the school nurse prior to the clinic or come to the clinic with a parent or guardian to provide consent. If the student comes to the clinic alone we can call a parent to get verbal consent.  We will also need something to verify the childs age and, if at all possible, some form of photo id. 

For age verification we can accept the following:

Drivers license
Non-drivers ID
Birth certificate
Passport
Insurance document that contains a birth date
Marriage license
Picture ID can be either drivers license, passport, or school ID.

to pre-register for an appointment, or we will be accepting walk-ins/those with no appointment. We do not need written consent for this vaccine because those receiving Moderna must be 18 years old or older.  Dont worry the registration system will not allow anyone younger than 18 to register for this vaccine.
