A current physical is required for all new students, 7th grade, 10th grade and all student's participating in athletics. A school physical is performed in the health office by the school physician (Dr. Khan) or one of his associates. The physical is a brief and fairly routine checkup; eyes, ears, throat, heart, lungs, and hernias. If the school physician finds your child to be healthy and without any apparent concerns, that information will be put on your child’s school health record and you will not be contacted. If, however, the doctor has any concerns, I will contact you immediately for possible follow-up with your own family doctor. If your child has had a physical in the last calendar year, they will not be required to have a school physical. A letter explaining this procedure will be sent home to the parent/guardian of each individual student required to have a physical. If you would like your child to be examined by your personal family doctor in place of the school physical, please print out the attached form and take it to your doctor's office and have it faxed back to the health office at (716) 962-5788.
